Appointment Schedule:
Nurse regular attends to the patient as per assigned schedule, the patient can call at any time and make necessary appointment.
Nurse visit at premises:
Nurse check general patient health condition and adjust care as per doctors assigned medical plan, patient and their family will be informed about patient's health while doing all what is assigned per treatment plan, and complete all medical care documentation and make plan for the next visit.
Collaboration with the Family doctor and GP's Nurse:
Nurse regularly informs family doctor about patient's health condition and if needed make adjustments to the patient care plan.
