How the health care at home works

Health Care at Home include the following services:

  1. Giving Medicament (oral medicament distribution, subcutaneous injection, intramuscular injections, intravenously injections, i/v infusions);
  2. Skin damage health care (primary care to heal skin and soft tissues after damage, remove stitches, secondary bandage to heal the damage, bedsore caring, trophic ulcer care);
  3. Artificial opening care (colostomy, ileostomy, nefrostomy, cistostomy, tracheostomy, gastrostomy, urinary catheter care);
  4. Enteral feeding through probe;
  5. Blood test and other biologic material and tests collection for the laboratory;
  6. Vital signs performance testing;
  7. Education and training for patients and relatives about health care, health promotion activities and patient-care;
  8. Enemas pursuit;
  9. Healthcare specialist 24 h care at home and in the clinic;
  10. Other health and social care services.
Appointment Schedule:
  • Nurse regular attends to the patient as per assigned schedule, the patient can call at any time and make necessary appointment.
Nurse visit at premises:
  • Nurse check general patient health condition and adjust care as per doctors assigned medical plan, patient and their family will be informed about patient's health while doing all what is assigned per treatment plan, and complete all medical care documentation and make plan for the next visit.
Collaboration with the Family doctor and GP's Nurse:
  • Nurse regularly informs family doctor about patient's health condition and if needed make adjustments to the patient care plan.
